Dr Syed Mumtaz graduated from Dow Medical College in Karachi, Pakistan. He joined the practice on the 1st April 2008. He holds diplomas from the Royal College of General Practitioners and Physicians. His special interests are general medical diseases and cardiology.

Prescription Clerk
PCN Clinical Pharmacists
Clinical pharmacists work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.
Clinical pharmacists have a Masters degree or equivalent in Pharmacy followed by a year’s professional training to become a registered member of the General Pharmaceutical Council. They also undertake further training to equip them for General Practice work. Our PCN clinical pharmacists will be Independent Prescribers or working towards this qualification and work as part of the wider practice team, under the supervision of a GP.
Clinical pharmacists play a leading role in ensuring that patients with chronic diseases get the maximum benefits from their medicines, reduce waste and promote selfcare as part of a shared decision making consultation.
Having clinical pharmacists in GP practices means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time.

Not everything that affects our wellbeing needs a prescription. Our Wellbeing Team offers support shaped around one simple question: “What matters to me?”
If you’re living with low mood, stress, financial worries, poor housing, a long-term condition, or just finding things difficult – we’re here to help. Research shows that getting involved in the community, learning new skills, volunteering, or light exercise can really boost your wellbeing and confidence.
With nearly 40 years of combined experience, we offer support over a 12-week period – at home, over the phone, or in the community – wherever you feel most comfortable. Together, we’ll create a personalised Support Plan to help you reach your goals and connect with local services and activities.
Who We Are
Our team includes:
Two Social Prescribers, who focus on improving emotional and social wellbeing. They can support you with things like:
- ✅ Confidence building
- ✅ Reducing loneliness
- ✅ Employment or housing concerns
- ✅ Daily routines and healthy habits
- ✅ Connecting with local groups, activities and services
One Care Co-ordinator, who supports people living with long-term physical health conditions. This role is especially helpful if you’ve recently been diagnosed or are finding it hard to manage day-to-day. They can assist with:
- ✅ Understanding and managing your condition
- ✅ Accessing personal health budgets and appointments
- ✅ Care planning for short- and long-term needs
- ✅ Support for you and your carers, including training and information
- ✅ Working closely with your GP and other services to ensure joined-up care
